Coming home from the hospital

Care Alternatives is not affiliated with any hospital or rehabilitation facility; we are an independent Nurse Registry. What we can do is talk through the discharge instructions with you and refer caregivers suited to the level of help described — help with transfers, bathing, meals, and medication reminders during the first weeks home. We do not guarantee a placement timeframe.
Paying for care
Private pay is accepted. We also assist Palm Beach County families with the documentation long-term care insurers commonly request, including caregiver logs and service records, and we can point veterans and surviving spouses toward the VA-related home-care programs described on our Veterans page. Care Alternatives does not determine VA eligibility and cannot guarantee insurance reimbursement — those decisions rest with the VA and with your insurer.
